Here is a good paper put together by some guys at Synapticad.  It discusses the basics of creating a testbench with transaction level modeling.     The basic methodology proposed is  good.  The message of this site is to build on this foundation and show reuse across design stages, injection/collection logic,  scoreboarding where necessary. template base classes, etc.  click here
